ISO 22000 CCP and OPRP monitoring record template

The reading for each batch, at the moment it is taken and by whoever takes it. It is the most numerous record in the system and the first to give away a plan that exists only on paper.

In one sentence

The CCP and OPRP monitoring record notes, for each batch or interval, the value measured or observed against the critical limit or action criterion, the time, who monitored it and any deviation with its correction, and clause 8.5.4.3 of ISO 22000 requires it to be retained as evidence of monitoring.

Where the standard requires it
ISO 22000 · Clause 8.5.4.3 Clauses are cited using the numbering of ISO 22000:2018, the current edition. ISO is preparing a third edition that may renumber them; when it is published it will have its own transition period.
Type of information
Mandatory record Evidence of monitoring to be retained (8.5.4.3)
Who fills it in
The person responsible for monitoring designated in the plan They must be competent and know what to do in the event of a deviation
When
At the frequency set in the plan Per batch, per trolley, hourly or continuously
How long it is kept
At least the shelf life of the product It is the only period the standard sets (8.3). For long-life products that period governs; for fresh products it is worth extending it to the certification cycle.

What the standard requires

What ISO 22000 demands and what it leaves to your judgement

The monitoring system for each CCP and OPRP must define measurement methods or devices, frequency, responsibilities and records. For CCPs, monitoring must be capable of detecting any failure to meet the critical limit in time; where it is not continuous, the frequency must be sufficient to isolate the affected product.

The equipment used for monitoring must be calibrated or verified (8.7).

The standard does not say whether the record is on paper or electronic. It does imply that it must reflect what was measured and when: a sheet filled in at the end of the shift does not demonstrate monitoring.

Structure

Fields the template should include

  • CCP or OPRP and step
  • Date, time and batch or interval monitored
  • Value measured or observation
  • Reference critical limit or action criterion
  • Conforming or nonconforming
  • Correction applied if there is a deviation
  • Measuring equipment used
  • Person monitoring
  • Review of the record: who and when

Example

Example: monitoring of CCP-2, metal detector

Company, people and figures are fictitious: the example only shows what a properly completed record looks like.

Field Content
Point CCP-2 · Metal detector at the filler outlet
Date and batch 3 March 2026 · batch L26-062
Check Ferrous, non-ferrous and stainless steel test pieces passed at start-up, every hour and at the end
Result 09:00, 10:00 and 11:00: all three test pieces correctly rejected
Deviation 12:00: the stainless steel test piece is not rejected
Correction Line stopped; product since 11:00 held and rerun through the line 1 detector after it had been checked
Equipment Detector DM-02 · last manufacturer verification 15 January 2026
Monitored by Morning shift filler operator
Review Quality technician · 3 March, 15:30 · opens corrective action CA-26-014

At the audit

Mistakes auditors commonly flag

  • Records in the same handwriting and ink for the whole day, filled in after the event.
  • Deviations noted with no correction and no reference to the product held.
  • Monitoring equipment without current calibration or verification.
  • Nobody reviews the record, or it is reviewed by the same person who monitors.
  • Readings that are always identical and do not reflect the real variability of the process.

In Kimobox

How this record is kept in Kimobox

  1. 1The operator records the reading from the line terminal and the actual time is saved.
  2. 2A reading outside the limit automatically opens the correction and alerts quality.
  3. 3Each reading hangs off the batch, and traceability carries it along.
  4. 4If required, the daily close can be sealed on a blockchain so the evidence does not depend solely on the company's own system.

Frequently asked questions

What people ask us before getting started

Is an automatic equipment log acceptable?

Yes, and it is usually better than a manual one. What you must ensure is that someone reviews it at the planned frequency, that alarms reach whoever needs to act and that the equipment is calibrated.

Who can sign the review of the record?

Someone other than the person monitoring. The standard requires that verification is not carried out by the person responsible for monitoring that same activity, and reviewing records is one of the most common verification activities.
